Children under the age of 18

Minors are required to hold their own passport.

• a birth certificate of a minor (an original document and a copy / a verified copy by notary). If a parent or minor has changed his / her name after receiving the child’s birth certificate, he / she should provide documents verifying that, for example: marriage certificates / divorce certificates or certificate from Registry office (an original document and a copy / a verified copy by notary);

• a filled-in application form signed by both parents (question 37 and a last point on 4th page of the application form) along with a photo (photo requirements);

• a copy of a minor’s international passport’s main page;

• if minor is to travel with both parents, the following documents should be provided:
– a confirmation of an accommodation shared with the minor;
– round-trip tickets for the parents and the child;
– financial guarantees by any parent;

At presence of valid Schengen visas of the parents:
- a copies of the valid Schengen visas along with the main pages of the parents’ international passports;

At presence of a notarized exit permit:
- In the exit permit Schengen countries must be named as well as trip terms or the exit permit terms.

• if minor is to travel with one of his parents, the following documents should be provided:
– a confirmation of an accommodation shared with the minor;
– round-trip tickets for the parent and the child;
– a notarized exit permit signed by the second parent (both original and a copy / a verified copy by notary);
– the parent who signs the consent form should provide a copy of his / her passport’s main page;
– financial guarantees by any parent;

• if minor is to travel with a confidant, the following documents are required:
– a confirmation of an accommodation shared with the minor;
– round-trip tickets for the confidant and the child;
– a notarized exit permit signed by both parents (original documents and copies / a verified copy by notary);
– copies of both parents’ passports’ main pages;
– financial guarantees by any parent;

• if minor is to travel without any accompanying persons (16 y.o. or older), the following documents are required:
– a notarized exit permit signed by both parents (original documents and copies / a verified copy by notary);
– copies of both parents’ passports’ main pages;
– financial guarantees by any parent;

• if minor is to travel with his/her legal custodian, the following documents are required:
– a confirmation of an accommodation shared with the minor;
– round-trip tickets for the legal custodian and the child;
– documents providing the termination of parental rights or the loss of parents (original documents and copies);
– court decision on the appointment of a custodian;
– financial guarantees by legal custodian;

  • in case of a parent’s decease or termination of parental rights all the appropriate documents must be provided (originals and copies);
  • if the father’s name is mentioned by the mother choice in child’s birth certificate the reference form № 4 from the Registry Office must be provided;

• if a parent has gone missing or his whereabouts is not known, it is necessary to provide an appropriate police certificate;
